Key Responsibilities

• Maintaining continuity of care through the exchange of information with outside health and social agencies Serves as a liaison between primary care providers and specialists such as general surgeons.

• Consults with primary care providers on issues related to patient care Teaches and counsels individuals and groups with regard to specific health care needs.

Requirements

• hire, per state regulations Current active Mass DPH Controlled Substances Registration or ability to obtain post

• hire, per state regulations American Heart Association Basic Life Support (BLS) certification Preferred Qualification: Advanced Cardiac Life Support (ACLS) may be required based on specialty
