About the Role

Key Responsibilities

• will work collaboratively with other team members in providing high-quality, comprehensive primary medical care to Tri-County patients using best practice and evidence-based standards.

• will participate in coordinating care for patients, support patients and families in self-management and will engage in continuous quality improvement activities aimed at specific patient population needs.

• provide primary care services in our Federally Qualified Health Center (FQHC).

Requirements

• Graduate of an accredited master’s level Nurse Practitioner program; Current RI NPP License, Board Certified/Board eligible in Family Practice.

• Prior experience practicing in Patient Centered Medical Home (PCMH) preferred.
