Key Responsibilities
- as a Home Health Physical Therapist: Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
- Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
- Adjust treatment plans and techniques as needed to achieve optimal outcomes.
- Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
