About the Role

Key Responsibilities
- giving demands (i. e., Patient Care Volunteer, Home Health Aide, Respite). Educates patient and family about Advance Directives as a means of promoting self
- hours on call program and is available to respond in situations when the need for psychosocial care arises. Other duties may also be assigned.
Requirements
- EDUCATION and/or EXPERIENCE Master's degree in Social Work from an accredited school of social work.
- One (1) year of work experience in a healthcare setting required.
- Hospice experience preferred.
- CERTIFICATES, LICENSES, REGISTRATIONS Current and valid Tennessee driver’s license and automobile insurance as required by Tennessee State Law.

- Full-time position.
- Call responsibilities required.
