Key Responsibilities

Perform face-to-face intake for consumers who have been referred to MHC and assess for care management, medication management or therapy eligibility.

Complete all intake evaluation paperwork: initial intake, and grief when needed and any other necessary paperwork.

Determine an individual’s appropriateness for care management, medication management or therapy and assign to all that are appropriate.

Disposition consumer to appropriate Agency for other mental health services when ineligible for care management.

Requirements

Must be licensed with either with LPC/MHSP, LCSW or LMFT in the state of TN.

Valid Tennessee Driver’s License 4.

Acceptable Motor Vehicle Report (MVR) 5.

Benefits & Compensation

Salary range: $50,000 to $65,000 annually.

Signing bonus: $2,500.

Work Schedule

M-F 8-4:30 schedule.

No weekends required.
