As a Home Health Occupational Therapist, you will: Perform comprehensive functional assessments and contribute to interdisciplinary care planning with the RN, PT, patient, family, and physician.

Develop and implement patient-centered therapy plans focused on maximizing independence in daily living.

Conduct OASIS assessments accurately and in a timely manner.

Supervise and collaborate with COTAs and Home Health Aides, ensuring quality care and compliance.
