Key Responsibilities
- Support operational clinical workflows, provider services initiatives, and care coordination efforts to ensure continuity and quality of care.
- Deliver clinical services as appropriate via in‑person visits, telephone, or telehealth encounters across multiple care settings, including residential, facility‑based, and home environments.
- Serve as a clinical resource to interdisciplinary teams, supporting evidence‑based practice, quality improvement initiatives, and best‑practice standards.
Requirements
- Ability to perform a thorough assessment considering physical, cognitive, and psychosocial aspects.
- Ability to assess and diagnose acute and chronic illnesses and identify the risk factors associated with these conditions.
- Proficiency in evaluating functional status including activities of daily living (ADLs) and instrumental activities of daily living (IADLs). 2. Chronic Disease Management:
- Expertise in managing common chronic conditions in the adult and elderly populations (e. g., diabetes, hypertension, osteoarthritis).
