Key Responsibilities

Support operational clinical workflows, provider services initiatives, and care coordination efforts to ensure continuity and quality of care.

Deliver clinical services as appropriate via in‑person visits, telephone, or telehealth encounters across multiple care settings, including residential, facility‑based, and home environments.

Serve as a clinical resource to interdisciplinary teams, supporting evidence‑based practice, quality improvement initiatives, and best‑practice standards.

Requirements

Ability to perform a thorough assessment considering physical, cognitive, and psychosocial aspects.

Ability to assess and diagnose acute and chronic illnesses and identify the risk factors associated with these conditions.

Proficiency in evaluating functional status including activities of daily living (ADLs) and instrumental activities of daily living (IADLs). 2. Chronic Disease Management:

Expertise in managing common chronic conditions in the adult and elderly populations (e. g., diabetes, hypertension, osteoarthritis).
