Key Responsibilities

AND RESPONSIBILITIES Conduct comprehensive assessment of the patient’s nutritional status and diet history and screen for potential risks.

Assess provider’s orders and develop counseling intervention/service plan to address specific health risks or conditions.

Provide culturally competent education to patients and their families on nutritional needs and diets.

Collaborate on all plans and patient progress with multidisciplinary care teams and other community supports.
