Key Responsibilities

  • AND RESPONSIBILITIES Conduct comprehensive assessment of the patient’s nutritional status and diet history and screen for potential risks.
  • Assess provider’s orders and develop counseling intervention/service plan to address specific health risks or conditions.
  • Provide culturally competent education to patients and their families on nutritional needs and diets.
  • Collaborate on all plans and patient progress with multidisciplinary care teams and other community supports.
