About the Role

BAYADA Home Health Care is seeking a Per Diem Occupational Therapist to join the Sierra Vista Visits team in Arizona. Provide skilled occupational therapy in patients' homes.

Key Responsibilities

• Perform thorough patient evaluations.
• Develop individualized intervention plans.
• Assess home environments for hazards and equipment needs.
• Collaborate with nursing and multidisciplinary teams.
• Educate patients and families.
• Document care accurately.

Requirements

• Licensed Occupational Therapist in Arizona.
• Willingness to travel within assigned territory.
• Graduation from accredited occupational therapy program.
• Minimum 1 year experience preferred.
