Join IMAGE STRATEGIC PARTNERS as a Social Worker in United States. Job DetailsJob Location: Cura Tulsa - TULSA, OK 74135Job Shift: DayMission Statement: We will serve our patients and their family by providing for their physical, emotional, and spiritual comfort. We will serve our team members by shepherding them with the values and attitudes of our Company.

Key Responsibilities

Engage with patients/caregivers to develop rapport and build trust while you assess their psychosocial and emotional condition, as well as their care environment, during regular visits.

Adapts communication style to each individual and circumstance to provide an atmosphere that protects patient dignity.

Formulates individualized plans of care for patients based on assessed needs, strengths, limitations, and goals.

Includes patient and caregivers in the care planning process.

Requirements

Requirements: Masters or Bachelor's Degree in Social Work preferred.

Bachelors Degree in Psychology or Sociology will be considered.

Prefer One year of social work experience in a health care setting, however we will train the right person.

Current Driver's License, Proof of Current vehicle liability insurance.

Work Schedule

Full-time position.

No explicit call, weekend, or night shift information provided.
