Join Capitol View Transitional Care Center as a Pharmacist, MTM and Population Health - Burnsville in United States. Park Nicollet is looking to hire a Pharmacist to join our Medication Therapy Management team! Requirements

doctoral residency. Licensure/ Registration/ Certification:

Current Minnesota Pharmacist license and maintain that license in good standing. Knowledge, Skills, and Abilities:

Board Certified Ambulatory Care Pharmacist (BCACP) or Board

Certified Pharmacotherapy Specialist (BCPS) preferred.
