Requirements

Candidates must possess a Doctoral Degree from an accredited physical therapy program or a Bachelor’s/Master’s Degree obtained prior to 2016, along with a current Physical Therapist license in the State of Indiana. Foreign-trained applicants must have educationally equivalent coursework accepted by the Indiana PLA, and Basic Life Support certification through the AHA is required.
