Join Wyckoff Heights Medical Center as a Social Worker LMSW – Inpatient/Outpatient in United States.
Key Responsibilities
- Completes patient assessment that includes identification of needs in discharge planning within the timeframe set forth in departmental policy.
- Includes biopsychosocial, financial, medical and physical factors in the assessment in consideration with age specific issues and clinical impressions.
- Respects patient’s cultural, language, religious and spiritual values, beliefs, preferences and accommodates accordingly.
- Assesses patient's capacity for self
Requirements
- This position requires a minimum formal education of Master's Degree and a minimum of 5 years job
- related experience.
- Must possess LMSW Licensed by New York State.
- Experience in medical acute care setting is preferred including inpatient and outpatient social work.
