About the Role

Join Wyckoff Heights Medical Center as a Social Worker LMSW – Inpatient/Outpatient in United States.

Key Responsibilities

Completes patient assessment that includes identification of needs in discharge planning within the timeframe set forth in departmental policy.

Includes biopsychosocial, financial, medical and physical factors in the assessment in consideration with age specific issues and clinical impressions.

Respects patient’s cultural, language, religious and spiritual values, beliefs, preferences and accommodates accordingly.

Assesses patient's capacity for self

Requirements

This position requires a minimum formal education of Master's Degree and a minimum of 5 years job

related experience.

Must possess LMSW Licensed by New York State.

Experience in medical acute care setting is preferred including inpatient and outpatient social work.
