About the Role

Key Responsibilities
- The Occupational Therapist evaluates and then directly provides or supervises the provision of occupational therapy services to patients in their homes or residences.
- When functioning in the capacity of the Home Care Case Manager (HCCM), the Occupational Therapist is responsible for the provision and coordination of all components of a patient’s home care services.
- The Occupational Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.
- The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.

- 3 years of job
- directed and able to work with little supervision. Computer skills preferred. Home care experience preferred.
