Key Responsibilities

• coordinate resident care plan meetings and develop and execute resident care plans based upon resident needs fo.

• provide the resident and family assistance in understanding financial programs available through Medicaid/Me.

Requirements

• Bachelor's degree in Social Work or Human Services (sociology, Rehab Counseling, Psychology)

• Minimum one

• year experience as a Social Worker in a long

• term care setting (preferred)
