About the Role
Clinical Case Manager I/II providing comprehensive psychological and behavioral case management services within a high intensity treatment team. Focus on improving mental health consumers’ abilities to function in the community with smaller caseloads.
Key Responsibilities
- Assist clients with basic needs and benefits applications.
- Arrange transportation and community resource linkage.
- Provide psychosocial services to improve daily living skills.
- Maintain clinical documentation.
Requirements
- Bachelor’s degree for CMI, Master’s for CMII.
Benefits & Compensation
- $30.12/hr (CMI) to $31.25/hr (CMII).
- Annual salary approx. $62,650 - $65,000.
- Language differential $1-$2.50/hr for bilingual proficiency.
