About the Role
Join Trinity Health as a Nurse Practitioner in United States. Employment Type: Full time Shift: Day Shift Description: The primary purpose of your job position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s). Performs assessments in collaboration with the physician and as needed.
Key Responsibilities

- conduct assessments, develop care plans, and ensure participants receive comprehensive care while fostering.
- provide acute illness treatment.
- Conduct home visits when necessary.
Requirements
- Qualifications: Master's degree in Nursing with five (5) to seven (7) years of experience required preferably in long-term care.
- Must possess a current, unencumbered, active license to practice in the state of PO.
- CDS Certification and DEA Registration required.
- Board Certification by ANCC or AANP.

- Salary: $49-$61/hr
