About the Role

Join Providence Care as a Hospice Social Worker – LMSW, Rock Hill in United States.

Key Responsibilities

Participates in development and re-evaluation of POC for clients needing MSW.

Instructs and counsels the client and family in treating and coping with the social and emotional responses to illness.

Participates in discharge planning.

Participates in care coordination activities and act as resource to other team members in identification and resolution of client needs.

CP in South Carolina Preferred: Independently licensed LISW
