Key Responsibilities

  • will build strong, lasting relationships with patients—offering culturally responsive care, continuity, and education to help manage complex health conditions and promote maximum functioning.
  • must be accessible 4-5x weekly to South Weymouth Main St, and Foxborough Patriot Place practices Must travel to patient homes who are within 45 miles of MGB practices.
  • develop innovative programs that deliver care where and when it is needed most.
  • provide home-based geriatric assessment and care management for older adults aligned with Mass General Brigh.

Requirements

  • MA State License] Nurse Practitioner [NP] / Advanced Practice Registered Nurse [APRN] [MA State License) Experience as a nurse practitioner, 3
  • F Eastern Business hours South Region: must be accessible 4
