Key Responsibilities
- Develops and maintains daily activity schedules.
- Transports clients and families to appointments and/or community resources, when needed.
- Provides case management, care coordination, and informal counseling for clients and families enrolled in program.
- Evaluates and assesses the needs of the clients and families.
