About the Role

Join VNA & Hospice of the Southwest Region Inc as a Physical Therapist – Kids on the Move in United States. VNA & Hospice of the Southwest Region is currently seeking a full-time Physical Therapist (PT) for the Kids on The Move program in Rutland County. Key Responsibilities

• Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.

• Creates patient specific goals and ensures that the plan of care reflects logical interventions and advancement towards these goals.

• Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc.

Requirements

• Successful completion of an accredited Physical Therapy program required.

• Two years of clinical Physical Therapy experience preferred.

• Prior Home Care experience preferred Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency.

• Experienced with electronic medical records required.

Benefits & Compensation

• Salary: $83,782 - $125,674/year

• 401(k) with employer match.

• PSLF eligible (Public Service Loan Forgiveness).
