Hospital Liaison Social Worker

Core responsibilities: Acts as the primary liaison between the hospice organization and hospital systems to coordinate patient admissions and care. Responsible for conducting psychosocial assessments and collaborating with interdisciplinary teams to ensure adherence to the hospice Plan of Care.
