About the Role

Join Centerlight Healthcare as a Remote House Calls Nurse Practitioner providing virtual clinical oversight and care coordination for vulnerable patients in their homes across multiple states.

Key Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program.
• Manage chronic conditions including diabetes, hypertension, CHF, COPD, and geriatric syndromes.
• Perform care management assessments and follow-ups.
• Coordinate care with primary care providers and specialists.
• Document clinical decisions accurately in EMR.

Requirements

• Active NP license and national certification.
• Minimum 2+ years clinical NP experience.
• Experience in home health, geriatrics, or primary care preferred.
• Comfortable with telehealth platforms and remote monitoring.
