Key Responsibilities

• Assess the need for special medical equipment and/or supplies and arranges for procurement.
• Assists families, and other staff members with developing coping mechanisms to provide care in the home setting
• Assists families and other staff members with developing coping mechanisms in emergency situations.
• Understands and appropriately performs telephone triage/management.
• Makes appropriate referrals and collaborates with community agencies to provide services to patient and families
