Key Responsibilities

• will work collaboratively with the patients, family physicians, other team members to provide comprehensive care to patients of the New Canadians Health Centre.

• will assess patients, as well as develop, implement, and evaluate care plans for patients both directly and indirectly.

• will work to full scope of practice as an autonomous primary health care provider with an emphasis on providing evidence-based care, education, and guidance to patients with chronic disease, as well as supporting the successful transition of patients to community practices.

• provide comprehensive care to patients of the New Canadians Health Centre.

Requirements

• Master’s or equivalent in Nursing

• Minimum of three (3) years or more of related experience

• Advanced skills in caring for patient with chronic, complex, and/or specialized needs

• Current BLS CPR is required
