Key Responsibilities
- will work collaboratively with the patients, family physicians, other team members to provide comprehensive care to patients of the New Canadians Health Centre.
- will assess patients, as well as develop, implement, and evaluate care plans for patients both directly and indirectly.
- will work to full scope of practice as an autonomous primary health care provider with an emphasis on providing evidence-based care, education, and guidance to patients with chronic disease, as well as supporting the successful transition of patients to community practices.
- provide comprehensive care to patients of the New Canadians Health Centre.
Requirements
- Master’s or equivalent in Nursing
- Minimum of three (3) years or more of related experience
- Advanced skills in caring for patient with chronic, complex, and/or specialized needs
- Current BLS CPR is required
