Key Responsibilities
- Provide direct face-to-face patient care, progressive clinical pharmacy practice; med optimization supported by collaborative practice.
- Responsible for managing chronic disease patient panel with focus on full risk, high risk patients with uncontrolled chronic disease states.
- Chronic conditions: Diabetes, HTN, HLP, HF, anticoagulation, etc.
- PGY1 residency strongly.
