Job Description

Concord Hospital Medical Group is seeking a compassionate and collaborative experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Rheumatology team. This full-time position offers a balanced opportunity to practice in both Concord and Laconia, caring for patients across the Lakes Region and Capital Area of New Hampshire. This role is ideal for an experienced APP who values patient education, team-based care, and developing long-term relationships with patients managing chronic rheumatologic conditions.

## About the Role As part of our Rheumatology practice, you will care for patients with a wide range of autoimmune and inflammatory conditions, including but not limited to rheumatoid arthritis, lupus, psoriatic arthritis, gout, and vasculitis. You’ll work closely with experienced rheumatologists and a multidisciplinary care team to deliver high-quality, coordinated care.
