Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO), board certified or board eligible in specialization required.
- State of Ohio and DEA licensure required.
- Completion of an accredited residency in specialty required and a fellowship training program in sub-specialty required (where applicable).
