Key Responsibilities

• Collects data pertinent to the healthcare consumer's health or the situation from patient assessments in the home setting, telephonic contacts and/or related EMR information available.

• Analyzes the data to determine the issues and available resources to seek effective resolution.

Requirements

• JOB REQUIREMENTS Education Doctorate degree in Physical Therapy from an accredited school or education requirements commensurate with the date of graduation.

• Maintains continuing education requirements as mandated by applicable state professional standards.

• Certification / Registration / License Physical Therapy License in the State of applicable state or South Carolina must be licensed in the state upon which you work.

• Basic Life Support for Healthcare Provider certification from the American Heart Association required.

Benefits & Compensation

• Hourly rate: $61.65/hr.
