Key Responsibilities
- Engage in completing some Housing Focused Person Centered Plans with people referred.
- Continually monitor progress and modify the Housing Support Plan and Housing Support Crisis Plan as appropriate.
- Communicates changes in service needs and recommendations to Clinical Supervisor/Mental Health Professional, Case Manager, and rest of treatment team on an ongoing basis to coordinate care.
