Description We are seeking a compassionate and experienced Hospice RN Case Manager to join our dedicated team. In this role, you will provide comprehensive end-of-life care, coordinate patient-centered plans, and support families during a critical time.

Requirements

• Provide bereavement support and resources to patients' families and caregivers as needed.

• Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.

• Perform all other duties assigned by the Hospice Director or designated supervisor.

Benefits & Compensation

• Salary: $80K - $100K

• 401(k) with employer match.

Work Schedule

• Full-Time position.

• Call responsibilities required.

• No weekends required.

• Day shift only - no nights.
