About the Role

Join Advanced Home Health and Hospice of Arizona as a Hospice Registered Nurse (RN) Case Manager in United States. Description We are seeking a compassionate and experienced Hospice RN Case Manager to join our dedicated team. In this role, you will provide comprehensive end-of-life care, coordinate patient-centered plans, and support families during a critical time.

Provide bereavement support and resources to patients' families and caregivers as needed.

Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.

Perform all other duties assigned by the Hospice Director or designated supervisor.

Salary: $80K - $100K

Full-Time position.

Call responsibilities required.
