About the Role

Join Fairview Health Services as a Home Infusion Staff Pharmacist – Rotation Weekends & On-Call in [object Object].

Key Responsibilities

Perform initial patient evaluation as it pertains to the prescribed therapies.

Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy.

Obtain histories and apply these along with other patient data to optimize drug therapy management.

Work with other healthcare providers to evaluate patients to determine appropriateness for home IV therapy.
