Key Responsibilities

• Develops and implements nutrition care plans based on approved standards of care to meet assessed needs, including short

• term goals; evaluates effectiveness of care plan and counseling in meeting established goals and plans follow up care as needed.

• Collaborates with interdisciplinary care team and gives integrated nutrition care as needed.

• Maintains records of nutrition care, documents effectively and communicates pertinent nutrition data though documentation in the medical record in accordance with agency policies.

Requirements

• Bachelor’s Degree in Nutrition, Registered Dietitian (RD), Licensed Dietitian Nutritionist (LDN) in the state of Massachusetts required

• Certified Diabetes Educator (CDE) preferred

• 1 year of experience in a healthcare setting

• Ability to communicate in second language Spanish, Vietnamese or Portuguese preferred

Benefits & Compensation

• Salary: $64K - $96K

• Signing bonus: $1.

Work Schedule

• Part-Time position.

• Hybrid position - combination of remote and on-site.
