Nurse Practitioner, Adult and/or Gerontology

About the Role

CommuniCare Health seeks Nurse Practitioners passionate about adult and geriatric care in skilled nursing and assisted living settings. Work Monday to Friday, 8am-5pm, with no call or weekend requirements.

Key Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education and develop treatment plans.
• Order and interpret lab and diagnostic tests.
• Collaborate with interdisciplinary teams.
• Maintain compliant documentation.

Requirements

• Master's or Doctorate in Nursing Practice.
• National NP certification (AANP or ANCC).
• Active, unrestricted state license and DEA.
• Minimum 2 years clinical experience.
