Key Responsibilities

• Medically manages a defined caseload of geriatric patients in skilled nursing centers, assisted living or personal care centers, home health, hospice, or community settings.

• Makes periodic, pertinent patient visits to the center or other site of care.

• Provides appropriate, timely medical orders.

• Provides appropriate, timely and pertinent documentation and assessments.

Requirements

• Licensed as a Registered Professional Nurse meeting the Georgia Board of Nursing requirements to engage in advanced nursing practice and;

Work Schedule

• Full-time position.

• Call responsibilities required.
