  • & Responsibilities Provide physical therapy services in accordance with the plan of care established by the supervising Physical Therapist.
  • Administer therapeutic exercises, gait training, balance training, transfer training, and other skilled interventions.
  • Observe, document, and report patient responses to treatment and communicate progress or concerns to the supervising PT.
  • Accurately complete required documentation in the electronic medical record in accordance with agency policy and Medicare guidelines.
