About the Role

Join Healthforce as a Travel Physical Therapist – Hospital-Based in United States. Overview Physical Therapist travel assignment in Meeker, Colorado. This PT role supports hospital-based patient care with a start date of 04/27/2026 for a 13-week assignment.

Key Responsibilities

Assess patients and develop individualized physical therapy plans of care for a range of conditions.

Conduct initial evaluations, set goals, monitor progress, and document findings in the patient record.

Deliver PT interventions including therapeutic exercise, gait and balance training, manual therapy, and modalities as appropriate.

Reassess and adjust plans based on patient response; coordinate care with referring physicians, inpatient teams, and other therapists.

Requirements

Active Physical Therapist license (state

specific) in good standing

Ability to work in a hospital/acute care setting

Willingness to travel for a 13

Salary: $2,240 - $2,267/week
