About the Role
Join Healthforce as a Travel Physical Therapist – Hospital-Based in United States. Overview Physical Therapist travel assignment in Meeker, Colorado. This PT role supports hospital-based patient care with a start date of 04/27/2026 for a 13-week assignment.
Key Responsibilities
- Assess patients and develop individualized physical therapy plans of care for a range of conditions.
- Conduct initial evaluations, set goals, monitor progress, and document findings in the patient record.
- Deliver PT interventions including therapeutic exercise, gait and balance training, manual therapy, and modalities as appropriate.
- Reassess and adjust plans based on patient response; coordinate care with referring physicians, inpatient teams, and other therapists.
Requirements
- Active Physical Therapist license (state
- specific) in good standing
- Ability to work in a hospital/acute care setting
- Willingness to travel for a 13

- Salary: $2,240 - $2,267/week
Work Schedule
- Contract position.
- Day shift only - no nights.
