Key Responsibilities

Collaborate with a Supportive Team: Be an active and valued member of the interdisciplinary team (IDT), working together to coordinate and deliver holistic, participant-centered care.

Monitor Participant Wellness: Keep a caring eye on participants’ health by monitoring weight, hydration, dietary intake, and changes in chewing, swallowing, mood, or medical conditions.

Share updates with the team to ensure proactive care.

Requirements
