Key Responsibilities

• will be responsible for providing therapeutic services to patients dealing with a wide range of issues by counseling them and helping patients and their families navigate the medical system, assessing and monitoring patients' such as depression, anxiety, and eating disorders by counseling them on managing ongoing health conditions and making positive behavior changes.

• provide quality, patient-centered, holistic care—and build caring relationships.

Requirements

• Master’s Degree in social work is required.

• Bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.

• Experience Qualifications Prior experience in social work or past social service clinical experience directly transferrable to a healthcare environment is preferred.

• 2 years of home care, hospice, discharge planning or related experience is preferred.
