Key Responsibilities

Employs counseling skills in order to help patient/families understand, accept and follow recommendations for care and to restore patient to optimum social and health adjustment.

Stays up-to-date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.

Intervenes in crisis situations, and effectively applies crisis intervention theory techniques.

Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare network.
