Join Easterseals NY-Valhalla as a Social Worker – Summer Temporary in United States.

Key Responsibilities

Complete social service assessments (to include providing client information on Advanced Directives) within required timeframe of admission as ordered by referring physician.

Participate in the development and maintenance of the master problem list and social work treatment plans, and identify appropriate short and long

term social service goals for clients.

Update the master treatment plan based on the changes to the master problem list and social work treatment plans.
