Collaborates with physicians, nurses, and other healthcare professionals, providing education, communication and support for healthcare staff, patients, and families.

Work Schedule

Full-time position.

Requirements

Candidates must possess a Master's Degree or greater as a Nurse Practitioner from an accredited program, along with 3-5 years of medical experience, including at least one year as an NP and one year working with frail or elderly populations. Required licensure includes an NC NP license and ANCC Board Certification, along with the ability to function effectively within a team-based PACE model.
