Key Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries. Including performance of follow up exams if required.

• Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services.

• Provides services, which may include the ordering of appropriate diagnostic and laboratory tests, the prescription of pharmacological agents, and other non

Requirements

• Master's Degree Graduate of accredited master's level Family Nurse Practitioner or accredited Physician Assistant program Required Experience Qualifications

• 3 years work experience in a healthcare setting Preferred Skills and Abilities

• Demonstrate organizational skills that allow timely patient appointments and appropriate required follow up.

• Excellent interpersonal skills and behaviors
