About the Role

Join Pennant Group as a Occupational Therapist (OT) - Home Health - Walnut Creek, CA in United States. This is a home health role and will require the occupational therapist to work in the field visiting patients in the community. Key Responsibilities

AND RESPSONSIBILITY Provides patient, caregiver, and family counseling.

Provides patient and caregiver education.

Communicates with the physician who is responsible for the home health plan of care and other health care practitioners (as appropriate) related to the current home health plan of care.

Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.

Requirements

Valid CA OT License Possesses and maintains current CPR certification.

Muir Home Health is a joint venture between John Muir Health and Cornerstone Group which has about 75 home health and hospice agencies throughout the country.
