About the Role

Join Valence Care as a Speech Therapist – Home Health, Per Visit in United States. Speech Therapist - IDD/Geriatric Home Health (Per Visit / Fee for Service, approx. 10 visits/wk) Extended Home Care is looking for an experienced and reliable home care Speech Language Pathologist to conduct home visits in Yonkers and the surrounding area for our IDD and geriatric patients.

Key Responsibilities

Assesses patient’s speech and language abilities and deficits.

Prepares an initial assessment of the patient’s condition and perform periodic reassessments.

Develops a plan of care in accordance with the authorized practitioner’s plan of treatment for therapy services and according to agency policies.

The plan of care must include the type of services rendered, an estimated duration of treatment, the frequency of visits with clearly defined, measurable short and long term goals.

Requirements

$90 per visit
