About the Role

Join Olympia Fields Hospital as a Home Care Physical Therapist in United States. Key Responsibilities

Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals.

Communicates pertinent information verbally or written to physician, other staff members, patients/caregivers.

Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program.

Requirements

EDUCATION, EXPERIENCE, TRAINING 1.

Bachelor, Masters, or Doctorate Degree in Physical Therapy from an accredited program.

One year recent clinical experience.

Home Care experience preferred.

Salary: $74K - $108K
