About the Role
Join Permanente Health Care Ventures, PC as a Skilled Nursing Facility Physician in United States.
Key Responsibilities
- will care for a spectrum of medical complexities, ranging from straight-forward, elective orthopedic surgery patients to those with multiple, complex medical problems at each facility.
- will support our home health field staff, in members’ homes, as well as consults (physician consults to determine patient eligibility).
- will also support the “Transition Nurse Team” for escalations of patients transitioning from recent Hospitalist discharges or SNF discharges.

Benefits & Compensation
- Salary range: $298,000 to $298,000 annually.
- Signing bonus: $50,000.
