Key Responsibilities

• manage a case load of clients with mental health needs.

• provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability,.

• Conduct individual assessments, assessing functional strengths, resources and needs.

• Develop individualized, person-centered, trauma informed community support plans.

Requirements

• of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills.

• Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.

Benefits & Compensation

• Salary: $27 - $30/hr

• PSLF eligible (Public Service Loan Forgiveness).
