About the Role
Join VNS Health as a Senior Social Worker, Hospice Home Care, Manhattan in United States. Overview Provides clinical and consultative support to Licensed Social Workers (LSWs) and Interdisciplinary Care Team Managers regarding complex social work related clinical and/or customer service issues. Provides mentoring and leadership to less experienced LSWs.
Key Responsibilities
- provide direct social work services to patients and families in collaboration with the interdisciplinary car.
- Conduct field orientation and orientation co-visits for newly hired social workers.
- Provide supplemental support, coaching, and clinical consultation as needed for both new hires and incumbent.
- Conduct annual clinical co-visits for SWs within their assigned regions.
