Key Responsibilities

• Provide home

• based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury

• Work as part of a home health team which may include an RN, LPN, HHA, OT and SLP, focused on the patient’s plan of care and goals

• Observe movements, conduct assessments, document progress and modify care plan as needed

Requirements

• CPR certification

• Knowledge of state and federal home health regulations
